Males of Parablennius postoculomaculatus have more intense markings on the underside of the head, a round, pale-edged brown spot on the postorbital region.
They are characterized by numerous medium to blackish brown spots on the lower half of the cheek, the sides of the upper lip, the suborbital region, the branchiostegal membranes, the pectoral fin base, and the basal third of the pectoral fin.

On the upper 2/3 of the body there are 6 to 7 groups of elongated rows of dark spots.

Parablennius postoculomaculatus is a greenish-gray to pale orange slime fish with many dark spots, sometimes grouped into saddle-shaped, oval, or round spots interspersed with a brick-red spot, a large pale-edged brown spot behind the eye, and reddish-brown spots on the head and pectoral fin base.

Males have a dark spot on the fin membrane between the first and second dorsal fin spines.

Parablennius postoculomaculatus is an optional air breather.

Parablennius postoculomaculatus is called the false Tasmanian slime fish because its appearance and habitat are similar to those of the Tasmanian slime fish (Forsterygion gymnotum), but it does not belong to the same species.
